• Residency application information– if you submit information to apply to become a resident at our senior living community, you may be asked to provide information such as your contact details, Social Security Number, date of birth, annual income, proof of income, driver’s license or other government-issued ID number and details, health information relevant to care planning, emergency contact information, physician contacts, insurance information, prior residence history, reference contacts, and other information that is relevant to your application.
• Residence information – if you are a resident of our community, we will collect and maintain information regarding your residence, including emergency contacts, family members, your care preferences, health-related information, the specific details of your residence, and other information that may be described in our community rules and regulations or residency agreement as being collected and maintained by us.
• Marketing information– details regarding informational and promotional materials you may have requested or received from us, the senior living communities in which you are interested, your receipt of promotional communications, and information on your marketing or communication preferences.
• Communication information– copies of communications and inquiries you have submitted to us, including through email, calls, chatbots, and features available on our Services.
• Device and usage information–details regarding how and when you use our website, including the device used to connect to our website or software, your IP address and device identifier, the frequency and duration of your usage, the pages you view, what websites or search terms referred you to our website, and information about your interaction with our website.

We collect this information when you visit or navigate our website, fill in forms through use of our Services, submit residency applications or inquiries to us, review community information and submit payments to us, communicate with us (including by phone, email, or otherwise), visit or engage with our social media pages, participate in surveys or sponsored activities, or otherwise provide us with personal information.
